Smoky Mountain Post-Acute and Rehabilitation Cente emerged as Sevier County’s largest nursing home during the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility had 120 beds available, exceeding the Sevier County average by 11 beds as of the first quarter of 2026. It cared for an average of 81.7 residents throughout that time frame.

The CMS gave Smoky Mountain Post-Acute and Rehabilitation Cente an overall rating of 1 out of 5, which is below the Tennessee nursing home average of 3 reported statewide.

Sevierville Health and Rehabilitation Center ranked as the second largest nursing home in Sevier County for the quarter, with 98 available beds.

By the end of the first quarter of 2026, Smoky Mountain Post-Acute and Rehabilitation Cente had incurred $61,180 in fines from two penalty events and faced a total of three penalties.

There was only one for-profit nursing home operating in Sevier County, Tennessee during this period.

U.S. Census Bureau estimates indicate that the older adult population in the country is increasing, while the number of younger individuals is declining. From 2020 to 2024, Americans age 65 and older grew by 13%, whereas the population under 18 dropped by 1.7%. Over the last century, the nation’s older adult population expanded significantly, from 4.9 million in 1920 to over 61 million in 2024.

A 2021 study found about half of this demographic, or 28 million people, could need long-term services and support. The projection also noted that more than a third may eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education group. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
